POOR EDUCATION: It's frustrating to witness the failing education systems in some places. A lot of students end up skipping classes because the system is so corrupt, and as a result, they can't really defend their knowledge or skills, which leads to them being jobless.
FINANCIAL BURDENS: Some individuals genuinely try to get back on their feet, but the debt they accumulate just drags them down. Debt is like a ticket to poverty.
JOBLESSNESS: The unemployment rates are shocking. There are jobs available, but greedy people hoard these opportunities and only give them to their friends or family members, even if they aren’t the best fit for the role.
PHANTOM NAMES: This might sound strange to those unfamiliar, but in civil service, there are these "ghost names"...
The main reason everyone can not be rich is because of the lack of knowledge and exposure to what can make them become rich.
If everyone is rich, life can not be balanced, but it is not good to be poor. Some people are poor because they lack education.

I was surprised to see ghost names in your list. Anyway, it should have been classified as corruption. These ghost names in the civil service exist because some top government officials are corrupt. Corruption is one of the major causes of poverty in developing nations. When funds budgeted for developmental projects are pocketed by a few individuals, other citizens will lack the opportunity to come out of poverty.
Let me also add laziness as another cause of poverty. Some people have the opportunity to work but they choose to be idle because of laziness. They don't want to work but they want to satisfy their needs. You see people declaring to the state that they are unemployed so that they can claim benefits from the government while they remain unproductive.
It's about their personal choices in life. Even those who didn't have role models at home which they could have used as motivational examples, there is always someone in society who can fit that role. So I guess disfunctional families aren't an excuse at all to justify why most people don't manager to get rid of poverty. From what I observe, it's related to how those people spend their money.
Instead of saving and investing, they spend everything on ephemeral goods. In worst cases, alcohol, cigarettes, drugs, prostitution and games. There are also those who spend with products like clothes, accessories, snacks and digital gadgets.
Moreover, they end having children in the wrong moment of their lives, or having more children than they could really afford.
These are some factors, although there must be many more. It's also important to notice each case is unique, so there isn't a general answer to explain every cases.
If the country is led by a poor governance, majority of the citizens wont also take progress but will remain poor for the longest time. Most especially if all their needs and wants they just rely on it from the government.
Another reason is when people adopt the poor mindset of living. They stay lazy and unmotivated, so expect that they will stay poor forever. But if they will imitate how the rich process their mindset, maybe there wont be a lot of poor people living in a country. At least they should also learn to help theirselves before asking help and assistance from others.
It is easy to point accusing fingers at the government as a facilitator of poverty, but in truth, they are not the only reason. It should not be surprising but there are people who are the cause of their own poverty, and this is because of the choices they make or have made in the past. There are people that have made poor choices in life and others even when met with an opportunity to change and improve on their financial situation, totally ignored and skipped the required action to take advantage of the opportunities.
Personal decision is also a reason for poverty. If people learn to make better decision and become more proactive, poverty rate will reduce.
Because majority of the people just rely on the government. Note that no matter how rich a government is, if the people themselves do not want to work on their own lifes improvement, they will surely end up poor for the rest of their lives.
Also, lack of job opportunities and privileges can also be a great cause. They shouldnt wait for the government to do it, but find their own opportunities instead by building their own source of income.

To start with do you think some body can prefer leaving in complete poverty over good life, to be honest with you some where born with a silver spoons while others where not born with any spoons but struggle to make themselves who they are so because I strongly believe men are born but hero's are made if God is by your side.
Some may have the desire to go to school for academic qualification but ends up not getting because they don't have what it takes to make themselves who they want to be while some have finished in flying colour but ends up not getting employed even with there first class degree certificate. One of the main reasons why most poor people remain poor is that they are unable to do anything other than support their families due to the rising prices of essential goods due to the country's rising inflation. If a poor person spends all of their income at the end of the month, they cannot save enough to invest or establish another business, and as a result, they remain poor for generations. Besides, there is a lack of education, a lack of environment, a lack of facilities, etc., but the most terrible problem in his life is the country's economic problems.
I know the feeling when you saw people begging for money along the street, of course is always sympathetic. But sometimes I blame them because had it mean they struggle hard like other people definitely they wouldn't have find themselves in such situation. So I would say that thier laziness is what brought them into such condition, Alot of people think that for you to have a source of income that you must work in a company. Not knowing that they can actually make a source of income from skill, if there's lack of employment in the country of course those who are not opportune to work in the company need to struggle hard to set his or her own source of getting money. I dont want to say that some people are just destined to be poor like its their fate, but somehow if they dont do anything to make their lives prosper and create positive changes, then most possibly they chose to stay poor rather than to experience comfort and convenience in their way of living.
However, some factors may also be possible that majority of the people remain poor. Lack of education and skills, low wages and unemployment. With this, if the government cant guarantee lifes progress for them, its better if they will move on their own and work on their own path towards lifes success and financial stability.
Social security systems in most of the world have been widely investigated multiple times and the case of what OP called "ghost names" ok is nearly non existent. Turns out governments these days have good ways to track their population's life statusb especially when they are being given money. Once someone dies their social accounts get called into question and automatically terminated.
Therefore some serious crime would have to be committed by a third party to keep receiving payments. Body being hidden, identity fraud and lack of investigation by local authorities. An in person check at a persn's residence would not be that hard. These cases are nearly non existent because this is a fraud too hard and risky to execute for just a monthly payment of a few bucks.
What if the elites have made the system, like the educational system that made us believe that it's one ways of being rich?
And they're setting it in our young minds that the way to riches is to become successful educationally and then find a job from a company that they own?
Isn't this the normal system that we used to live with and until now, we believe this. While there's truth to it, what if there are other ways that they've discovered and not exposing it to the public for them to retain that for themselves?

The rate of poverty for me is connected to the level of education in the country, when there's education at his highest level then people will be more exposed to means of making money and majority will begin to get rich. Looking at well developed countries and those from underdeveloped, you can see the difference in wealth among their citizens and that's why I say education is one of the major reasons for people getting money the rest of the things you said play their parts but when we don't have education, we won't be able to understand how to hustle for money. The mindset of people also matter because when we don't have the mindset of being rich, we can't be rich and despite we winning the lottery, still we won't be capable of managing that money to retain our wealth but waste it all.

Or do you have a solution for a government like Cuba where the main cause is sanctions from superpowers like the United States? Or do you have any solutions for the Republic of Congo, Niger, Benin, the African countries that are still being manipulated and plundered by France?
Depending on the cause of poverty and the circumstances they face, there will be different solutions. Poverty is a problem that has existed for thousands of years in human history, and it is not as easy to solve as many people think.
There's no formal education system that teaches you how to get rich or at least direct you to a viable source of income. All the material is practical education commonly found in life, career, or work. Of course, you have to find your own path.
That's the bottom line: no one finds a gold mine and then shouts to everyone to show them where it is. The formal education system teaches you what to do when you find yours.
It's a statistical fact that most children are born into poor families and poor countries. From birth, they are deprived of access to quality education and a nurturing environment for well-rounded development. As a result, they get little schooling and are forced to start working at a young age.

Your assertion is correct. It's so disheartening to see the high level of poverty in the society toady. It's crystal clear that majority of the world population are living in adject poverty especially in the developed countries and this can be attributed to so many factors such as poor planning, diseases, corruption, laziness, inadequate information, crises, war, poor educational system, high population, lack of innovations.
Among all the attributed factors, the major root of high rate of poverty in any society is corruption and poor planning, if you critically look at those countries that have high rate of poverty you will notice that corruption is very in those countries.
After corruption the second factor that is causing high rate of poverty in the society is poor planning, some people don't know how to plan, for instance, some people live extravagant life, the hardly save money to make little investment.
